The Family Educational Rights and Privacy Act (FERPA) affords students certain rights with respect to their education records including Parental Access to Student Information and Release of Information to a Third Party. 

 

You must complete and submit the FERPA Consent form to authorize Nassau BOCES administrators to discuss your educational records.  Print the form by clicking on the link below and follow the instructions.  No information will be released until Nassau BOCES Adult Evening Program is in possession of a fully completed, signed and notarized form as per the instructions. 

 

Parents requesting information on dependant students must complete parts A-H (notary not required) and prove dependant status by submitting Page 1 of the last filed Federal or State tax return.  The dependant student must be listed.  All financial information and social security numbers should be blanked out.
